All students who participate in DCSD athletic programs are required by the GHSA to have a completed and up-to-date Student-Athlete Medical Packet on file with the Lakeside High School Athletic Department before they can participate in any sports activity.  Please note that NO ATHLETE WILL PRACTICE OR PARTICIPATE IN COMPETITION UNTIL ALL FORMS ARE COMPLETE.

All LAKESIDE students should complete a profile under “LAKESIDE HIGH SCHOOL.”

What is DragonFlyMAX: Dekalb County School District utilizes DragonflyMAX as our new electronic medical records system.  Below you will find instructions on how to set up your athlete’s profile and upload any required documents e.g. physical form. This information is kept on a HIPAA and FERPA compliant server providing the utmost secure storage for medical information.

Why DragonFlyMAX: Dragonfly Max is an integral part of the athletic healthcare delivered at your child’s high school. Our athletic trainer will review each athlete’s physical and when necessary, apply appropriate medical precautions. The messaging capabilities of the App will enable athletes, parents, coaches, physicians and athletic trainers to all be on the same page regarding an athlete’s injury or illness. This will expedite referrals, return to play decisions, and encourage athlete compliance with home instructions.

● Parents will need to login/create an account and add/update your players info prior to practice.
● Physicals are valid for one calendar year.  If you are a returning Lakeside player, please check the date on your physical to make sure it is still current.  All forms are uploaded in Dragonfly Max and can be printed out for your convenience.
